I created a fillable form to be used when initiating a cigar pass. It can be confusing when starting a pass, to know exactly what information is required. Please take a look at the PDF I created. This can be filled out using your web browser, and then printed once complete.

Let me know if I'm missing anything...

The forms need a dedicated column for cigar "number" on the original list and the P&T list.

I would add a section to page 1 on "Shipping rules and instructions" because people seem to fuck it up every time. Nothing as fun as having your pass go to a burned down warehouse or sit in a PO in new mexico for a month because somebody didn't bother to confirm an address etc.

In the participants list, I'd add a specific area for a contact number(s).

I made all changes requested except for this one. I just added numbers to the original cigar list, however when people place in PUTS, those won't be numbered, so I don't see a point in even numbering the original list. Unless I'm missing something...

Puts are usually numbered as they are put in, added at the end of the list.

That's the thing... I'm going to have a tough time numbering puts if I don't know how many will be put in.

I will upload the newest version later tonight and see what you guys think. Some of this stuff, like numbering puts, may need to be done by hand individually.

Looks good. Would name, handle, and address fit in one box? Then you could have more names on one sheet for the pass order sheets. You would get 16 people on one sheet compared to 8.

It could just be Chrome but the default font size is enormous.

Start typing the form and the font will automatically resize itself to fit.
