If in fact it was real it's natural for us to wonder why nobody did anything to him any sooner, but unless a person has had training or plenty of experience in totally unexpected and "fight or flight" moments, odds are that most people in a group setting wouldn't quite know how to react in the heat of the moment, or else they just plain wouldn't.

Just goes to show that a good cigar is the best therapy going. Too bad he didnt know that.
