While smoking presents a concern for increase risk of cancer in the long term, the regions of the world mentioned, seem to me at least, to have more pressing short term concerns. Who cares if you get cancer when your sixty if the local warlord kills you when your 24.
As a side note on Bruce's comment, its not just smoking and such, I remember reading an article a few years back about Zaire or some other nation, turning down wheat and other crops because some UN scientist told them that said crops were geneticially engineered. Which is absolutely true, engineered to be drought and insect resistant. If your drowning and someone throws you a rope do you really care about the enviromental impact of the production of it.
