PHP/Mysql Guru

I need help on creating a php search query into a mysql database. I have one written that is very simple that can take a single input and search multiple fields. What I need to do is have a 4 or 5 field form where you can enter data into one or more fields and then match that against the database. It is beyond my skill set but I imagine that an experienced fellow could probably do it in 15 minutes. Heck, you may even have a stock script laying around that I can tweak with a pointer or two.

I'm familiar with both. Shoot me what you got in a PM, along with some psuedo-code of what your trying to do (write it out as best you can - don't worry about syntax or correct code at all).
