Cigar Review Journal Template/Ideas

I would like to start keeping a cigar journal with short reviews to go back to in order to see which cigars I have enjoyed and why. And I'm sure down the line to see how my taste has changed/evolved. I am very new to smoking cigars and have been told by many seasoned smokers that the best way to start out is try a bunch of different cigars and figure out what you like. If anyone has a good template to share or some ideas of what I should keep in my cigar log I would greatly appreciate it. I am not looking to write a long review on each cigar. I want to be able to enjoy the cigar and then as an after thought write down some quick notes. Thanks, guys.
 
In the interest of full disclosure I did not "invent" this, I got it mostly from Cigars for dummies a LONG time ago.
 
I use a google docs spreadsheet (see the link in my signature). I don't log every cigar I smoke but I try to, especially if it's something new. This format allows me to write as little, or as much as I'd like. But the real advantages for me is that I can edit it from my android phone while I'm smoking, I can sort the data to see how different stick stack up against each other, and I don't have a pile of paper work laying around.
 
I started using an inexpensive journal from Barnes and Noble. I would glue the band to the top of the page, then add the date, some information about the cigar, and any other notes to help remind me about the cigar in the future.

I don't really keep the journal anymore though.
 
Thank you all for the help. I was thinking I might just do the journal thing from B&N but use some of the criteria from the templates.

Unrelated to the journal: I also want to find a few good short cigars to try. I want something that I can smoke in 30-45 minutes. I love to enjoy a longer smoke but during busy times it is tough for me to find a good hour or hour and a half to have a nice smoke. Any suggestions?

One last thing, I would like to find a decent cigar that is mild and on the inexpensive side. I have had some friends who are not cigar smokers visit lately and offered cigars and then watched them smoke less than half and put it out. Now, I do not mind sharing good cigars but it can be hard to watch a good cigar go to waste. Any suggestions on an inexpensive cigar for a newbie that would still be a good experience? Something I could just have a few of in my humidor.

I saw these featured in CA a few weeks ago, I actually had 3 of them in a cart ready to buy but I didn't pull the trigger because it was almost $50 with shipping. But I see on the link you posted it shows 3 packs and that makes it a lot better I didn't know they were sold like that I thought I was getting 1 booklet per order. I went back and did find that on the ordering website so I must have overlooked it. ???
